Faith Formation/Sacrament Registration

To register for People of Faith Gatherings, Sacrament Preparation,
Children's Faith Formation, Junior Youth Group or Senior Youth Group,
please fill in the form below and submit with your payment.

Offerings and Fees for Faith Formation this Year:

Adults:	People of Faith Gatherings	$30/person **
High School Students:	People of Faith Gatherings with Family _{(For Confirmation candidates whose families do not attend} ^{People of Faith Gatherings, Confirmands & sponsors may attend together)}	$30/person **
	Senior Youth Group (Grades 9 - 12):	$20
	Confirmation Preparation (10^th & 11^th grade) (Meetings & retreat)	$75
Middle School Students:	People of Faith Gatherings with Family	$30/person **
	Junior Youth Group (Grades 6 - 8):	$20
Elementary Students (PK - 5):	People of Faith Gatherings with Family (includes "People of Faith Children")	$30/person **
	Harcourt “Call to Faith” text book for grades 1 – 5 (in addition to the People of Faith Gathering Fee)	$20
	1^st Reconciliation Preparation (2^nd or 3^rd grade)	$20
	1^st Eucharist Preparation (2^nd or 3^rd grade)	$20

** People of Faith Gatherings are $30 per person. Total Faith Formation fees will not exceed $200 per family.

No one will be denied registration due to an inability to pay.
Please contact Christa Blomstrom or Eileen Mazary if your family has difficulty
paying for any of our Faith Formation programs.
We ask families to pay what they can, when they can.
