People of Faith/Sacrament Registration

To register for People of Faith Gatherings, Sacrament Preparation,
Children's Faith Formation, Junior Youth Group or Senior Youth Group,
please fill in the form below and submit with your payment.

Families who want to register their children for Children’s Faith Formation, Junior Youth Group
or Senior Youth Group may do so by registering their family for “People of Faith”
and selecting the appropriate Faith Formation level for each child. Also, remember
to note the size of your family in the “Fees” section.

To also register your child for Sacrament Preparation (1st Reconciliation, 1st Eucharist, and
Confirmation Years 1 & 2), you should select the sacrament each child is preparing for.
